School Physicals for Fifth Graders

* On January 6 and 7, 2005, Steve Ritchins, PA-C, from the School Doctor’s office in Guilford, will be at PCMS to do physical exams for children in grade 5. We are one of only a few schools in the state that offer this service to our students and their parents free of charge. This is a wonderful opportunity for a well child checkup as well as preparation for participation in sports next year in grade 6. Please look for a permission slip in your child’s papers. You are always welcome to accompany your child during this physical. Please call me with any questions.

Sue Pulkkinen, School Nurse, 876-4633

Please send back the form, even if you don’t want your child to have the physical.
